I'm not sure I can help you directly, but we are also running a dual-monitor with HMD setup. The best way we've found to set it up (recommended by the vizard guys) is to unattach the secondary monitor in the Windows control panel, then set NVidia to Horizontal Span. This makes Windows think it's running a single doublewide monitor, but apparently it's more processor-efficient than setting two monitors. Vizard then (using viz.stereo) splits the image and sends one to each half of the "doublewide" monitor, or one to each physical monitor. Note that we have a dual-channel video card, and each monitor is still attached to its respective output.

Also, we've noticed that our HMD (a Kaiser SR80A) can only handle its native resolution of 1280x1024; this may be why you're not getting any display in the HMD at higher resolution. Let me know if I can be of any more help!
