In order to make the original head disappear you...

In order to make the original head disappear you can comment out the line in the cfg file that defines the mesh for the head.

I am not quite sure what you mean by a 2D head. If this is just a picture of a face you could create a texture quad and apply the face image to it. Then you could link the texture quad to the neck...
